We can fix that...

Squib bullet blocking the barrel is my guess. Actually one of my fears in reloading. I pay extra attention to unusual sounding rounds.

It looks to me like the problem was between the cylinder and the barrel because that's where the explosion was, otherwise the barrel might have split.
It also doesn't quite look like current Rugers. Rear sight's not quite right. Not a Kimber or Taurus, AFAIK.
