Every gun produced in mass has had fails like that.
BUT.....I know of only a very few isolated instances where a few bad guns made it past QA from a major brand and they were quickly recalled.
Almost exclusively this is stupid people, playing stupid games and winning stupid prizes.
Reloading to excessive pressures, modifying parts when they do not have a clue, shooting the wrong cartridge in a gun, shooting after a squib or other barrel obstructions.

I have done some commercial gunsmithing and know that it is almost impossible to have a catastrophic failure with a factory fun and factory ammo. I have also seen blazen stupid and fixed many guns after someone modified them to the point they were dangerous.

The only exception to that is a few firearms made by garage shop inventors and they will have horrible reputations if you just Google them.
Old School
The best upgrade for you firearm is always instruction and practice.
